血浆ACTH、血清皮质醇测定对肾上腺皮质功能减退的诊断价值

Value of plasma ACTH and serum cortisol in diagnosis of adrenal failure

本文报告原发肾上腺皮质功能减退（PAF）患者13例、继发肾上腺皮质功能减退（SAF）患者31例及正常人42名血浆ACTH及血清皮质醇测定的结果。

The levels of plasma ACTH and serum cortisol were observed in 13 patients with primary adrenal failure , 31 patients with secondary adrenal failure , and 42 healthy persons .

结果表明，皮质醇和UFC水平降低是绝大多数肾上腺皮质功能减退症病人的特征。

The results suggested that the low levels of cortisol and UFC were a common feature in the most of patients both with primary or secondary adrenal insufficiency .

结论DAX1基因突变是导致儿童原发性肾上腺皮质功能减退的常见分子病因，而SF1突变较罕见；

Conclusion DAX1 gene mutations may be a frequent cause of primary adrenal insufficiency in children , while mutation in SF1 seldomly occurs .

继发性肾上腺皮质功能减退患者多数人血浆ACTH正常，而血清皮质醇和（或）UFC降低，他们对胰岛素低血糖试验或氨基导眠能试验无反应，对ACTH刺激试验呈延迟或正常反应。

In most of secondary adrenal insufficiency the ACTH levels were in normal range but the cortisol and UFC levels were low ; the elevated action of ACTH by insulin induced hypoglycemia test or aminoglutethimide test did not appear in the patients with secondary adrenal insufficiency .
